I confirm that if my child is on medication it is my responsibility to handover the medication to a designated volunteer with the necessary instructions.

I consent to Camp CoEJ volunteer administering any medication which is supplied in its original container and is clearly marked with the dosage and frequency. This is in accordance with the Camp CoEJ policy.

I consent to any emergency medical treatment necessary during the course of the camp. To the best of my knowledge, my child is medically fit to take part in the activities planned.

All campers are to ensure that they listen to all the safety instructions from the mentors and the core support team. All campers are expected to follow all the rules and regulations provided by the mentors and the core support team. All campers are to abide by the Islamic Code of Dressing.
